Heart To Heart provides a wide spectrum of health and social services throughout NY and NJ, including home care, adult day care, behavioral health, and a wide variety of related services to individuals of all ages within the community. Since 2006, Heart To Heart has provided Home Health Aide and Personal Care Aide services through contracts with most of the Certified Home Health Agencies (CHHAs) and Long Term Home Health Care Programs (LTHHCPs) in New York.

Heart To Heart's flagship division of Home Care has Accreditation with Commendation from the Joint Commission on Accreditation of Healthcare Organizations. As a result, Heart To Heart has distinguished itself by obtaining contracts to provide many specialty programs. Heart To Heart has invested in technologies to support its management vision. We have developed and continue to expand capabilities in management systems and cutting-edge technologies necessary to improve the quality of care.
