Phoenix Behavioral Health

Our mission is to provide a path to wellness and recovery for individuals struggling with mental health and substance abuse difficulties. Our caring professionals utilize a person-centered, holistic treatment approach that incorporates evidence-based treatment with compassionate care.

We offer a wide range of services including partial care mental health services and addiction treatment for adults ages 18 and over.

Our holistic approach includes on-site psychiatric care, engaging evidence-based groups, and person-centered treatment. Transportation and meals are provided. Medicaid accepted.

We provide ISS (Individual Support Services), which is direct care in the client's home and community. This includes personal assistance with toileting, eating, bathing, transferring, dressing, and personal hygiene. Our services also offer parents and caregivers relief from assisting in these personal tasks for up to 12 hours per week. 

For clients between the ages of 16-21 who are looking for more independence as they transition into adulthood, we offer teaching and mentoring to learn life skills such as money management, social skills, medication self-administration, housekeeping and grocery shopping, and accessing public transportation. This is a new service that can be requested through PerformCare or your Case Manager.
